Office ManagerSamantha, a South Bend, Indiana native, has built a distinguished career in the dental field, spanning over 20 years. She is a proud graduate of Indiana University South Bend’s dental assisting program, where she laid the foundation for her extensive expertise.
Throughout her career, Samantha has gained valuable experience working both chairside and at the front desk, allowing her to develop a comprehensive understanding of patient care and office management. Her dedication to fostering strong relationships with patients is evident in the way she connects with them during each visit, ensuring they feel comfortable and valued.
Beyond her professional life, Samantha cherishes her time spent with her son, family, and friends. Whether she’s enjoying family gatherings or engaging in community activities, she is deeply committed to maintaining close ties with her loved ones. Samantha’s combination of professional skill, genuine warmth, and a strong commitment to patient care makes her an integral part of our dental team.

She takes great pride in providing exceptional care to her patients and educating them on the importance of maintaining optimal oral health. Her approach to patient care is both thorough and compassionate, ensuring that everyone she treats feels informed and comfortable during their visit.
Jennifer is happily married and is the proud mother of two beautiful daughters. Outside of her professional life, she enjoys traveling and exploring new places, with a special interest in discovering unique breweries. Family time is precious to her, and she cherishes moments spent with loved ones.

Missy
Dental HygienistMissy, an esteemed graduate of Indiana University South Bend, has dedicated 20 years of her career to working with Dr. Kosek. Throughout her tenure, she has greatly valued the familial atmosphere fostered within the team, contributing significantly to the warm and welcoming environment of our office. Her role extends beyond routine dental duties; she is instrumental in educating our patients about oral health, ensuring they receive comprehensive care and guidance.
Outside of her professional life, Missy enjoys spending quality time with her husband and two children. Her ability to balance a demanding career with her family life exemplifies her commitment and dedication both at work and at home. Missy's long-standing service and personal approach to patient care make her an integral part of our dental team.
Carolyn
Dental HygienistFrom an early age, Carolyn developed an interest in dentistry, influenced by her positive experiences with orthodontics as a child. She began her career path as a dental assistant during her senior year of high school, later advancing her skills at Indiana University South Bend (IUSB), where Carolyn obtained her Certified Dental Assistant (CDA) certification with expanded functions, being part of one of the first classes trained to place restorations. After a brief career break to raise her children and explore real estate, Carolyn returned to IUSB to earn her Bachelor of Science in Dental Hygiene (BSDH) and obtained an Anesthesia license.
Her professional experience includes working in pediatric dentistry, with a focus on children and special needs patients, ensuring they have positive dental experiences. Married for 43 years to George, also known as Skip, they have two sons, Jake and Ian. Jake and his wife Rachel recently celebrated the birth of their first child, Luke. Ian, who is mildly autistic, has successfully launched a greeting card business. In Carolyn’s leisure time, she enjoys quilting, traveling to quilt shows, and engaging in outdoor activities like camping and kayaking with her family.
Jenny
Dental HygienistJenny Floyd is a dedicated dental professional who has been a valued member of Dr. Kosek's team for decades, officially joining as a full-time employee over four years ago. With nearly 25 years of experience as a licensed dental hygienist in the area, Jenny has a long-standing commitment to providing exceptional care to her patients. She earned her Associate of Science in Dental Hygiene in 1999 and a Bachelor of General Studies degree in 2000 from Indiana University South Bend (IUSB).
Jenny began her career offering temporary dental hygiene services to numerous local practices before taking a full-time position at a Neighborhood Clinic in Southeast South Bend, where she served the underprivileged community. Her passion for education led her back to IUSB, where she taught a course in ethics and jurisprudence, as well as provided clinical instruction to the next generation of dental hygienists. In addition to her teaching and community service, Jenny has gained experience working in several private dental practices over the years.
Jenny is happily married to her husband, Jack, and is the proud mother of three sons: Christian, Jackson, and Jenson. Her family also includes a German Shepherd/Husky mix named Max and a pet rat named Ricky. Outside of work, she enjoys spending time outdoors and dancing to live music. Jenny is excited to be part of the dental family at Dr. Kosek's practice and looks forward to welcoming you with her warm, friendly demeanor.
Melissa
Expanded Functions Dental AssistantMelissa has over 20 years of experience in the dental field and is deeply committed to the continuous learning that her profession demands. As a dedicated dental assistant, she is passionate about providing exceptional care to her patients. Melissa brings valuable expertise in a wide range of dental procedures and techniques, and her friendly, meticulous approach fosters a welcoming environment for patients.
She excels in anticipating the needs of both the dentist and patients, ensuring that operations run smoothly. Whether assisting during complex procedures or educating patients on oral hygiene, Melissa’s commitment and expertise are evident in all that she does. Her unique blend of technical skill and genuine empathy makes her an invaluable asset to our dental practice.
In her spare time, Melissa enjoys spending time with her two children and her beloved pet, and she has a strong passion for baseball.
Stormi
Expanded Functions Dental AssistantStormi is a highly experienced Expanded Functions Dental Assistant with over 37 years of expertise in the field. She has been a dedicated member of Dr. Kosek's team since his first day in practice in August 1996. Passionate about her work, Stormi takes great pride in helping patients improve their smiles and enhance the functionality of their teeth.
She is committed to staying current with the latest advancements in dental technology, eagerly learning and integrating new techniques into her daily practice. While she enjoys all aspects of dentistry, Stormi specializes in implants and orthodontics, including both traditional braces and tray aligners.
Born and raised in South Bend, IN, Stormi is a proud mother of three grown daughters and a loving grandmother to two young grandsons, with whom she cherishes spending time. Outside of her professional life, Stormi enjoys gardening, landscaping, fishing, DIY home improvement projects, and spending quality time with family and friends.

Business AssistantKelly, who joined our team from Las Vegas, Nevada, brings over 20 years of extensive experience in the dental field to her role as our business assistant. Her career encompasses a comprehensive range of dental specialties, providing her with a deep and versatile understanding of the industry. This wealth of knowledge and experience significantly enhances the efficiency and effectiveness of our office operations.
Kelly has been married for over a decade and is a devoted mother to a young son. Her passion for beauty and fashion is evident, and she enjoys indulging in these interests during her leisure time. Additionally, Kelly values spending quality time with her family and friends, balancing her professional commitments with her personal life.
